Individualized Service Plan (ISP) Management

“Consumer-Centered” Individualized Service Plans (ISP) is the core function of our service coordinators. During ISP visit, we establish a services monitoring plan to ensure all services are being delivered in the type, scope, duration, amount, and frequency as authorized and approved in the ISP.

Providing Choices for consumers is essential. Not only is it important that consumers are informed of their choices, it has to be documented that the choice was offered. All providers must respect the wishes of the consumer as long as it does not place the welfare and safety of them and others at risk.

While we do take consideration to the preference of the caregiver we as a company will make contact at least once per month to assure all services are being delivered in the type, scope, duration, amount, and frequency as authorized and approved in the Individual Service Plan.

We schedule monthly or quarterly visits or monthly or bi-monthly phone calls, to ensure that health and safety are maintained and the implemented plan meets the needs, goals, and preferences of our clients.

A Vast Array Of RESOURCES

Since the plan participant’s goals vary, it’s important for us to have a wide range of resources to accommodate their needs. In addition to medical treatment, housing, employment, volunteering, and transportation, we provide ongoing assistance in gaining access to needed State Plans and HCBS services, as well as needed medical, social, educational, and other services, regardless of the funding source.

Protect Consumers Interest

We follow guidelines recommended by the Office of Long Term Living to assure all of our staff is in compliance. During all of our ISP visits. We also ensure our consumers and their families are Educated about OLTL programs, services, and participant rights and responsibilities by reviewing with them the participant information materials developed by OLTL in order to help ensure their health and safety.
